Care Plan Review Meeting Template for Australian Home Care
What to include in a care plan review meeting note for Australian home care and aged care providers.
Care plan review notes help providers track whether supports still match the person's needs, preferences, goals, and risks. They also help coordinators record what was discussed and what needs to change.
A practical review meeting template should include attendees, current services, what is working well, changes noticed, preferences raised by the person or representative, risks, agreed actions, responsible staff member, and next review date.
Example wording: 'Client reported domestic assistance remains helpful but requested morning visits where possible. Daughter raised concern about increased fatigue after appointments. Coordinator to review visit times and monitor fatigue notes over next four weeks.'
In aged care, documentation should support person-centred services and clear follow-up. The strengthened Aged Care Quality Standards have applied from 1 November 2025 and place emphasis on safe, quality care that meets needs and preferences.
This type of template is operational. It should be adapted to your provider's assessment, review, consent, privacy, and escalation processes.
