NDIS Progress Note Examples for Support Workers

A practical guide to NDIS progress notes, case notes, and support logs for Australian disability support workers.

NDIS providers need clear records of supports delivered to participants. The NDIS record keeping guidance refers to records such as invoices, support logs, rosters, case notes, and service agreements.

For support workers, a useful NDIS progress note connects the activity to the participant's support and goals without over-writing. It should identify what support was delivered, how long it took where relevant, what the participant did, and what should happen next.

Example wording: 'Supported Alex to attend local library and practise public transport skills. Alex checked the timetable using his phone with one verbal prompt and tapped on independently. Goal link: building confidence using community transport. Plan next session: practise return trip with reduced prompting.'

Case notes may include progress and plans for future sessions. Keep the note factual and do not copy sensitive details into the wrong system. Follow your organisation's privacy, storage, and escalation process.

If the note is connected to a claim, roster, invoice, or support log, make sure the information is consistent across records.
